We accept applications year-round and schedule interview on an on-going basis.
 We accept referrals. Referrals can come from a social worker, case manager, family member, school, court, hospital, or self-referral.  Rose Compassionate will review basic information about the potential resident (age, background, needs).  
  - A phone/ zoom or in person interview or form is completed to determine if the individual meets the home’s eligibility criteria (e.g., age range, diagnosis, gender, legal status).
  
  - We will request documents like:
  
  - Psychological/psychiatric evaluations
  
  - Medical history
  
  - Educational records
  
  - Legal documentation (e.g., court orders, guardianship)
 
The Next Step is an in-person Assessment
  - A formal assessment with the potential resident (and family/guardian, if applicable).
  
  - This may include:
  
  - Clinical assessment by a therapist or clinician
  
  - Behavioral assessment
  
  - Observation for fit with current residents and program
 
  
If accepted, an admission plan is created, covering:
  - Intake date
  
  - Assigned case manager or counselor
  
  - Individualized care or treatment plan
  
  - Medication management plan is created
 
 
The following will be required:
  - Signed consent forms
  
  - Medical clearance
  
  - Immunization records
  
  - Guardianship or custody papers
  
  - Insurance information
 
Final step of admission
 
  - On the day of admission:
 
    
      -  Tour our group homes
  
      - Introduction to staff and other residents
  
      - Review our house rules and expectations
  
      - Completion of any outstanding documents
  
      - Personal belongings will be checked
